The assignment: Using Survey Monkey (or another software of your choice), create a three-question survey on a subject that is of interest to you. Your survey should have each of the following question types:

“Rating Scale”

“Comment/Essay Box” (click on single raw questions box)

“Multiple choice (only one answer)” with “yes” and “no” answers

After you create the survey, collect at least 25 responses. You must have at least 25 respondents for this assignment, but more is OK. Do not go over 40 participants, since the Survey Monkey website may ask you to pay. If some recipients do not respond, you must either re-contact them or send the survey to someone else so that you get 25 responses. After you received at least at least 25 responses to the survey, it is time to write the report. Your report should contain thoughtful responses to the following questions and meet all applicable requirements.
