Need HTML code for a simple file upload form where the user enters his email address in a field, then he uploads a file. The file will then be sent to my email address (e.g. ABC@ZYZ.COM) together with the text showing the entered email address. Please note that the website accepts only html code. 

Alternatively, do you know of a free domain where they allow file upload
